We will always try our best to verify insurance prior to your visit; however, you are responsible to verify that our office is a covered provider within your network.

  1. Picture ID - Your records are kept by the name on your picture ID

  2. Insurance Card - Name on insurance card must match your ID

  3. Method of Payment - We collect copayments and deductibles at every visit
